In-Home Support Service

What We Do:
We provide assistance and care in your home, a family member's home, or within your local community. Our aim is to support individuals with intellectual disabilities in various aspects of their daily lives, ensuring their well-being and safety.
 

Services We Offer:

  • Skills Development: We help individuals acquire, maintain, or enhance essential self-help, social, and adaptive skills needed for independent living.

  • Daily Assistance: We offer routine support for day-to-day activities to make life easier and more manageable.

  • Safety Measures: Our services include safety support to ensure a secure and worry-free environment for our clients.
     

Who We Can Help:
We're here to assist individuals with intellectual disabilities to lead a fulfilling life. This support can be provided to up to three individuals simultaneously, such as siblings. However, remember that the reimbursement rate per person decreases as more people are included. We also have backup plans in place for times when regular in-home support cannot be provided as scheduled.

Personal Assistance

What We Do:
Our personal assistance services are all about helping you with daily tasks and activities that make life smoother. We're here to support you with activities like getting dressed, going out in the community, taking your medications, and maintaining your overall well-being. We can provide this help either at home or in community settings to ensure you can live independently and participate in community activities.
 

How We Work:
Our personal assistance services can be managed in two ways:

  • Consumer/Self-Directed: You can take the lead in choosing the assistance you need with the help of a Services Facilitator.

  • Agency-Directed: We can provide you with a personal assistant to take care of your needs.
     

Backup Plan:
We believe in being prepared. So, we'll help you and your family or caregiver create a backup plan in case your personal assistant can't make it to work as expected or leave the job without notice.

Where We Provide Care: We offer our services at your home or in community settings, ensuring you receive the care you need where you're most comfortable.
 

What We Can Do for You:
Our services cover a wide range of activities, including but not limited to:

  • Monitoring your health status.

  • Assisting with medication administration.

  • Supporting you with other medical treatments.

We're here to help you maintain your independence and ensure your well-being at home or in the community.

Respite

What We Do:
Our respite services offer temporary, substitute care for individuals who usually rely on family or unpaid caregivers. We step in during times of emergency or when the primary caregiver needs some well-deserved rest. This care can be provided at home or in the community to maintain overall health and essential life skills.
 

How It Works:
Respite services can be managed in two ways:

  • Consumer/Self-Directed: You have control and can choose the support you need with the help of a Services Facilitator.

  • Agency-Directed: We provide a dedicated caregiver to take care of your needs.

We're here to give family caregivers a well-deserved break and ensure individuals continue to thrive at home or in the community.

Community Engagement

What We Do:
Our community engagement services help individuals acquire, improve, or maintain the skills they need for positive social interactions, greater independence, employability, and personal choices. We enable participation in typical community activities and functions, just like everyone else.
 

How It Works:
Our activities are designed to build relationships and natural support systems within the community, using everyday settings and interactions with individuals without disabilities.
 

Allowable Activities Include:

  • Community events and activities

  • Educational and cultural experiences

  • Volunteer opportunities

  • Employment readiness activities

  • Staying in touch with family and friends

  • Skill-building and self-direction through community collaborations

We're here to help individuals build social and life skills, fostering independence and community engagement.

Group Home Residential Services

What We Do:
Our group home residential services offer skill-building, routine support, general assistance, and safety support in a licensed or approved residence. These services help individuals acquire and maintain essential life skills necessary for successful living in home and community-based settings.
 

When It's Needed:
Group home residential services are authorized when individuals require these services beyond what's covered in their room and board arrangements with the service provider.
 

How It Works:

  • Services can be provided round-the-clock (up to 24 hours per day) by paid staff.

  • They may be provided to an individual or multiple individuals living in the same home, based on their specific needs.

  • Expect a focus on skill development and general health and safety support.

Our group home residential services aim to enhance essential life skills and safety for individuals in community living settings.

Companion Services

What We Do:
Our companion services offer nonmedical care, socialization, and support to adults aged 18 and older. We bring this service to you, whether it's in the comfort of your home or at various community locations.

 

How We Support You:

We focus on activities that enhance your daily life, including but not limited to:

- Assistance with tasks like meal preparation, laundry, and shopping.

- Support with light housekeeping tasks.

- Aid in self-administration of medication.

- Assistance with community access and recreational activities.

- Ensuring your safety is our priority.

 

What Makes Us Different:

Companion services are distinct from personal assistance and residential support. We don't provide routine support with activities of daily living like toileting, bathing, dressing, and grooming. Instead, our focus is on "instrumental activities of daily living" such as meal preparation, shopping, and community integration.

 

How It Works:

- Companion services can be managed in two ways:

  - **Self-Directed:** You have the autonomy to choose the support you need.

  - **Agency-Directed:** We provide a dedicated companion to assist you.

 

We're here to make your daily life more enjoyable and provide the companionship you deserve.
