Open Position- Direct Support Professional & Requirements Supervised By Program Assistant

​

• Direct Care Professionals must have at least six (6) months of experience in the ID/DD field. Direct Care Professionals may substitute this experience with a degree from an accredited institution in a related field. Related fields include, but are not limited to, Social Work, Sociology, Psychology, Nursing, Medical, Counseling, or any Human Services Field.

• Direct Care Professionals will be responsible for providing direct care services to our consumers based on the goals and objectives identified in a specific individual service plan. Some of the Direct Care Professionals' job duties are as follows, supporting individuals with room cleaning, sweeping, mopping, emptying trash, etc., meal preparation, diaper changing, toileting, bathing, laundry completion, community & social integrations, exercise routines, communication skills, personal money management, etc

• The Program Administrator will directly supervise direct Care Professional.
   

• Prior to employment, the Program Administrator shall require the following from each potential Direct Care Professional:

1. 3 verifiable references

2. A completed employment application for this program

3. State Driver’s License

4. A Social Security Card

5. Proof of car insurance

6. A recent copy of a negative tuberculosis skin test or x-ray

7. A recent copy of the individual’s Department of Motor Vehicles driving record.

8. First Aid and CPR certifications copies of any educational or professional degrees or certifications.

9. Completed and returned documentation verifying the individual’s criminal history and registry check findings.

10. Proof of previous experience, at least 6 months experience, working with the MH / MR population.
